There is no approved form of complaint. Here is a list of what must be indicated in the text of the claim:
Details of the organization to whom the complaint is addressed: name, legal address, full name of the manager.
Sender's details: your full name, passport details, contact phone number.
In the descriptive part, state the essence of the claim, what exactly you are complaining about, what you have already done (for example, called the dispatcher several times, sent a claim to the director), what you ask/demand to do. Here it is useful to refer to specific points of the agreement between the owner and the management company that were violated.
